Upload your two IDs

Form 1583 needs one current photo ID and a second document that shows your home address, each with a photo or scan.

USPS asks for one photo ID and a second document that shows your home address. Both must be current. Social Security cards, birth certificates and credit cards are not accepted.

Add each ID

On your form, under Your two IDs, there is a box for Photo ID and one for Second ID or proof of address. For each:

  1. Step 1Under Kind, choose the document.
  2. Step 2Fill in Issued by and Last 4 characters of its number.
  3. Step 3Enter the date under Expires. It is required for the photo ID; for the second document it reads Expires (if it does).
  4. Step 4Select Save.
  5. Step 5Select Photo or scan and choose the file. It uploads at once and shows Uploaded, with View to check it and Replace to change it.

Note: Files can be JPEG, PNG, WebP or PDF, up to 10 MB. An expired ID is refused: USPS requires a current one.

What counts

IDAccepted kinds
Photo IDDriver's license, state ID card, U.S. passport, military or uniformed services ID, USCIS card, certificate of citizenship or naturalization, tribal ID, NEXUS card, Matrícula Consular, foreign passport.
Second ID or proof of addressAny of the above, or a current lease, mortgage, deed of trust, voter registration card, vehicle registration, home or vehicle insurance policy, or Form I-94.

The address on your second ID must match the home address under About you. The mail center checks this.

Who sees your IDs

Only you and the mail center verifying your form can see the images. Once the mail center has entered your form in the USPS database, the images are deleted and only the ID kind, issuer, expiry and last four characters are kept.
