01 · What it is

The form that authorizes a mail center to receive your mail

PS Form 1583, Application for Delivery of Mail Through Agent, lets a Commercial Mail Receiving Agency accept mail on your behalf. A business lists the names mail is received for.

  • One form per person receiving mail
  • Your home address and your business details
  • The names mail may arrive under

02 · Identification

Two current IDs

USPS asks for one photo ID and one document that shows your address. Both must be current; you upload them in Holuway Address.

  • Photo ID: a driver's license or state ID, passport, military ID and others USPS lists
  • Address: a lease, mortgage, deed, voter or vehicle registration, or insurance policy
  • Social Security cards, birth certificates and credit cards are not accepted

03 · Signature

Signed in front of the mail center or a notary

USPS requires the signature to be witnessed. Choose in person at the mail center, a video call with the mail center, or a notary, and the mail center verifies the form.

  • The mail center enters it in the USPS database
  • Reminders before an ID expires
  • Mail is kept for you while you wait, and released once verified

Your address

A street address, written the way USPS wants it

Your address is the mail center's street address with your own private mailbox number. Holuway prints it in the exact USPS format, ready to copy, with a proof-of-address letter you can print.

  • Your box number (PMB) on the street line, as USPS requires
  • Copy it in one tap for your website, invoices and registrations
  • A printable proof-of-address letter from the mail center

Questions

Can I sign PS Form 1583 online?

You fill it in online, but USPS requires the signature to be witnessed in person or on a live video call with the mail center, or by a notary. A picture of a signed form is not enough.

What happens to my ID images?

They are kept in private storage and seen only by the mail center verifying your form. Once the mail center has entered your form in the USPS database, the images are deleted and only the ID type, issuer, expiry and last four characters are kept.

Does everyone in my business need a form?

Each person who receives mail at the address files their own form. A business lists the names mail is received for on its form.
