Choose how your form is witnessed
USPS requires your Form 1583 signature to be witnessed: in person at the mail center, on a video call, or by a notary.
USPS requires the signature on PS Form 1583 to be witnessed. You choose how when you sign, under How your signature is witnessed. Video call is selected until you choose another.
The three ways
| Choice | What happens |
|---|---|
| In person | Visit the mail center during its hours with both IDs. The pickup hours are on the location page. |
| Video call | The mail center schedules a short live video call and emails you the link. |
| Notary | Sign before a U.S. notary and give the notarized form to the mail center. |
A video call
- Step 1Sign and send the form with Video call chosen.
- Step 2When the mail center schedules the call, you get an email, and your form shows Your video check is on with the day and time.
- Step 3At that time, select Join the call on the form, or the link in the email. Have both IDs with you.
A notary
- Step 1Choose Notary when you sign, then select Sign and send.
- Step 2Select Print your form for the notary. The page sets out your answers in the form's order, with a line for your signature and room for the notary's certificate. Select Print.
- Step 3Take it and both IDs to a notary commissioned in a U.S. state, territory or D.C., and sign it in front of them.
- Step 4Give the notarized pages to your mail center. If the mail center or notary asks for USPS's own PS Form 1583, the print page links to it; copy your answers onto it.
Whichever way you choose, the mail center checks your IDs against the form and records how the signature was witnessed when it verifies the form.
